Definition.
Genito-urinary pain can be due to obstruction of hollow organs, inflammation, distention of the capsule (e.g., kidney, prostate) or infiltration of adjacent nerves. Acute obstruction to hollow structures leads to colicky pain which is very severe. Obstruction that develops over a prolonged period will lead to dull ache (e.g. obstruction due to malignant disease). The pain experienced is mainly due to the increase in smooth muscle contraction of the ureter trying to overcome the obstruction. Since this contraction occurs in waves the pain experienced is also in periods. This smooth muscle contraction is mediated by prostaglandins.
Aetiology.
Calculus
Blood clots
Necrosed papillae
Surgical (accidental) ligation of ureter
